Does the callsonline view exist, and does the user RadiusNT is
running as have sufficient permissions for it?

Most likely the problem is that the NAS-Identifier attribute from
the accounting record isn't matching an Address and port combination
from the CallsOnline view. You can run RadiusNT in -x15 debug mode
to see the NAS-Identifier and NAS-port Attributes and check for
matching entries in callsonline (which is derived from Servers and
ServerPorts table).
