Failure on Accounting packets

This is like Morton's Salt... "When it Rains it Pours"

I have a USR TotalControl HiPer ARC chassis that is sending accounting
packets to RadiusNT, but from some reason it thinks they are duplicate and
refused to add them to calls. Here is an example... Please note that it
did actually accept 1 and then the next 93 and now the next 140 are all
rejected? What is wrong?!!!!
