[Emerald] Incremental? and Summary?

I only have a handful of monthly overage type (Credit Card) accounts but
would like to get this working properly. I cannot get the system to
summarize the overages.

Emerald Admin 2.5.323
Emerald 2.5.331

i.e. I have a Dial UP called Big Ten:

$10/mo for 10 hours.
$1.50 / hr above ten.
Billed by CC on the 1st of each month.

Rates:
Overtime checked
Primary not checked
Standard Time 10
Standard Interval 3600
Standard Charge $0.00
Over Interval 3600
Over Charge $1.50

Client/Accounting

I recently unchecked Incremental
Calc past two decimals is unchecked
E-Mail Electornic (their spelling) Bill Receipts is checked.
No minimums.

Any ideas? I consistently get no consolidation needed when I try to run
it on the first of each month.
