[RadiusNT] Another Drwatson of a long username and another bug in RadiusNT

Note that RadiusNT does not convert and ' characters in the login string to
'' before trying to insert into the database, and that a long username once
again generates a DrWatson.

This is now really bugging me, as RadCheck does not re-start the service
properly after this.
