In IIS4's Management Console, right-click on your servername under Console
Root / Internet Info Server and choose Properties.
Click on the File Types button.
If the following don't exist add them:
Extension: mid
Mime: audio/x-midi
and
Extension: midi
Mime: audio/x-midi

I don't remember if you have to stop/restart IIS or not.
