Not sure about IIS4, but in IIS3, do the following:
Run "regedt32"
Go to:

HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Services\InetInfo\Parameters\Mim
emap
Click on Edit and choose Add Value:
audio/x-midi,mid,,>
Data Type:
REG_SZ
String Value: (blank)
Click on Edit and choose Add Value (again):
audio/x-midi,midi,,>
Data Type:
REG_SZ
String Value: (blank)
Exit the registry editor.
Stop IIS WWW service, then restart it.
