SQL not authenticating

Hello
I have a max 4004 and have configured it for radius authentication and
pointing it to the right ip address.
I have configured emerald and set up the sql database, and radius nt to
work together in the odbc authentication mode.
when i run the -x15 command from the radius directory it connects to the
odbc and everything appears to be running...
waiting for requests.
when i tried to test out a user name to get authenticated i am getting
this response:

What does this mean? and how can i get authenticated from the sql
database is it not working? Please help with what to do
