Have you tried this command?
ip radius source-interface Ethernet0/0
We had a problem where the routers would send the wrong ip address of some
other interface. I dont know if this pertains to you or not, but here it is
anyway.

I've seen this /w an old 2500...

Ended up rebooting to get the cisco to send the correct NASIdentifiers
again. You might be able to get around a reboot by resetting AAA.
