This is a pretty common platform.
I included the reply from IEA which is actually what I expected them to say.
If anyone has any suggestions please feel free. I am pursuing the white
rabbit (3com) too.
I will post my findings here, ultimately.

RADIUS accounting records are associated with the AcctSessionID, among other
things. The AcctSessionID is sent to RadiusNT from your servers. It sounds
like your servers are recycling their AcctSessoinIDs every time they are
rebooted. When this happens, you will start getting the duplicate entries
errors on your Calls table.
You should contact support for the vendor of your servers (i.e. Cisco, 3COM,
PortMaster, etc), and find out if the AcctSessionID is recycled on every
reboot and how you can avoid that.
