Had same problems, moved the order of the table as to your directions and was able to get
the table to have duplicates and be a primary key. Have done steps #1, #2 and #3
repeated times. It's recreating the relationships now. If I check the 3 boxes as normal
relationships have I get an No unique index found on the reference field of the primary
table". So I'm trying with "Enforce Referential Integrity" etc.
