This means we already receives the accounting record. If its happening
a lot, you should try and find out why its happening. This week I
helped
a customer resolve this problem when the secret configured on the NAS
for accounting was not the same secret configured in Emerald, which was
causing the NAS to ignore the ACK RadiusNT sent and re-send the
accounting
request.

This means RadiusNT already logged that authentication error for that
user for the current minute. This prevents your RadLogs table from
being populated with multiple entries when a user tries to authenticate
several times in a row (like when they are expired).
