Then I would guess that you RadAttributes table is not correct.
Check your primary key, and see if it is on the RadAttributeID,
RadVendorID, and RadValue fields.

There is a script at:

ftp://ftp.iea-software.com/RadiusNT/MSSQL/r25vsa_updt.sql

That can update an SQL database to include VSA support if you have
the old non-VSA table formats.
