[RadiusNT] Radius .175 crashing *after* accounting record

We are receiving authentication requests and accounting records from some
kind of Cisco device (we are getting more info on the HW/SW details) at a
third-party site which are causing Dr. Watson popups, *after* receiving a
"STOP" record, successfully saving it the database and stating that it is
returning an "ACK".

I can attach or send separately a capture of the -X15 output (user connects
then hangs up) and one or more Dr. Watson logs (both are short). The
exception is "access denied" apparently on a call to
"NtWaitForSingleObject".

We started with .124 and upgraded directly to .175, but the behavior has
remained the same. Any ideas?

(Ignore my earlier request for help with the database problem--I'll be
reinstalling SQL Server on that machine before we try it again).
