Check the spelling/caps of your username column in your Calls table
and make sure it matches the spelling/caps of your username attribute
(#1) in your RadAttributes table. The insert statement is dynamically
created based on what attributes in the request match columns in your
Calls table. In -x15 debug mode, RadiusNT will show you the columns
it found in your calls table.
