The RadLogs table will only record the same error for the same
person once per minute. Most dialup PAP/CHAP software re-tries three
times on a password failure. Therefore you'll typically get two of
there errors (although the first does get recorded).

The other message that you might see if the duplicate accounting
records. The second of the three accounting stats is the duplicate.
In you example above, thats 67. Most likely the ack that RadiusNT
is sending is either two slow (check the timeout on the NAS) or
something else is wrong (like incorrect secret).
