[Emerald] 287 invoice screen and master screen errors

On my system, When viewing invoices, they always say "renewal" even if the
invoice has been voided. If you try to void the invoice, it says all lines
already voided.

Also, even when the account has be deactivated on the master screen. Exit
account and return, active always shows as checked. An sql query shows
active = 0.

Finally, this is a permissions thing I think. Some machines (not users)
when saving credit card info, the name is replaced with a 1 when you exit
the name field in the CC box. The logins are set to standard and not
trusted. This occurs even when logged in as "sa" on the afflicted machines.
