I know that it's possible to use RadiusNT Enterprise version to forward auth
requests to another organization's radius server. But is there any way to
forward radius auth requests based on realm name style logins e.g.
MYREALM/<login> rather than <login>@mydomain.net?

Not currently. It wouldn't be difficult to add support for this is
a customer wanted it, though.
