This error usually means that RadiusNT already received the accounting
request. I have had reports of upgrading the OS of a USR and it
resetting the Acct-Session-ID counter, which caused all the duplicates
for a long time. If you are seeing this, you can update all the
Acct-Session-ID fields in your DB like:

Update Calls
Set AcctSessionID = '99' + AcctSessionID
