The active fields need to be non zero (standard boolean). I believe
there is a typo in the RadiusNT 2.2 docs about this.

Set saExpireDate to NULL. Also make sure your Extension and Limit fields are
set to zero as well. Are you using MS Access or SQL Server? If you are
using ODBC 3.0 or higher and SQL Server, make sure the "use regional date
and numbers" setting is unchecked.
