I'm not sure I follow you. What does the portmaster have to do with it?
Someone accessing the web site from the internet is not going through the
NAS and if I dial into the ISP it works fine because at that point I'm on
the internal network. Please explain.

The problem isn't IIS most likely he hasn't set the correct gateway in his
NAS. That's all
