I use post.office, which successfully blocks relay mail 100% of the time,
provided you
have your relay mail configuration set up properly. When I was still using
netscape MTA
I was constantly hit by spam relay mail. Once to the point where my server was
flooded
so badly that I was down for two days recovering it.

Post.office 3.x is fairly bug free and user friendly.
