These are the heart of the problem. Check your RADIUS Accounting timeout
value on the max and make sure its atleast five second. There is some
reason why the max isn't liking/receiving RadiusNT's ack in time.

You can also update your call trigger to prevent the phantom user,
but you should solve the above issue first.
