RadiusNT needs four things in an accounting packet to allow for
accurate accounting and concurrency control. All NASes that I
know of send ATLEAST these, and many other things.

1. UserName
2. Acct-Session-ID
3. NAS-Identifier or NAS-IP-Address
4. NAS-Port

These are the four fields in the database which make up the
key for the calls table.

Additionally, Acct-Status-Type, Acct-Session-Time and any of the
Acct-...-Octets attributes to track traffic.
