I did post the information earlier. The problem is that USR is sending
BOTH attribs
4 and 32 (NAS-IP-Address and NAS-Identifier). The RFC spec says one OR
the other.
The default dictionary has these both called NAS-Identifier, so that no
matter which
one is sent, its stored in the NAS-Identifier database field.

If both attribs are sent, you get a double NAS-Identifier in the INSERT
statement,
and it fails. To solve the problem, simply rename Attribute 32 to
something else.
