The Active column is set to 1 in both the MasterAccounts and SubAccounts =
tables. I have tried executing Radius both with and without the -I =
option. STILL no luck...

Is it possible I am having trouble because I have Radius and Emerald on =
one drive (drive c) and MS SQL 6.5 on another (drive d - both local =
drives)?

My guess is that one of the active flags are not set. Do you have
the case insensitive flag on? The M might be causing a problem.
